POSITION OVERVIEW
The Community Health Representative (CHR) is responsible for participating in and providing high-standard health prevention, treatment, and surveillance in the community.
Key Responsibilities
Research, publish, and distribute health education and promotion materials related to all areas of health
Educate the community on available health programs, health policies, and procedures
Coordinate the Good Food Box and Kitchen Workshops
Assist the Community Health Nurse (CHN) in delivering immunization and health clinics
Support community members with monitoring their health and taking preventative measures
Visit client homes to provide health education and maintain health surveillance
Assist patients in seeking and applying for services through other resource agencies
Assist in tracking client information for health center professionals
Provide clear direction to applicants regarding health programs, benefit access instruction, benefit delivery, and claims processing procedures
Ensure coordination of patient travel/accommodation arrangements for medical appointments and execute payment procedures for patient travel claims in a timely manner
Perform other related duties and reporting as required
QUALIFICATIONS & REQUIREMENTS
Experience working with First Nations people and communities
Valid BC Driver’s Licence and reliable vehicle
ASSIST, WHMIS, Food Safe
Completion of health-related education preferred
First Aid & CPR (preferred but not required)
Criminal Record/Vulnerable Sector Check
Ability to be flexible to meet changing priorities with good written and communications skills
TERMS OF EMPLOYMENT
This is a full-time permanent position located at Cheslatta Carrier Nation on the Southside. Compensation is competitive and commensurate to education and experience.
